Medication Refills
Medication refills are done at the time of your visit.
Online Scheduling
Patients can schedule, reschedule or cancel (atleast 24 hours before) an appointment at any time (24/7) using the "SCHEDULE APPOINTMENT online" button above. If unable to make an appointment online or thru the automated phone system please call

and leave a message for an appointment.
New patients should go to the New Patients tab and email or call our office. You can then use the onlline and automated scheduling features.
Medical Records
There is a form filling/faxing fee of $30 per form. Please remit this amount via mail to our office. Please make checks payable to Putcha Associates.
Also please EMAIL (to records@drputcha.com) consent for release of medical records
form (found in the "New Patients" tab above) .
Allow 5 business days for processing after remittance and consent is received.
Please DO NOT call or fax questions related to Medical Records. All such inquires MUST be through email.
Payments and Fees
Payment is collected in full at the time of your visit. We accept cash for all patients and checks for established patients only.
Please review our financial agreement page under the new patient tab. Patients are responsible for payment of missed or cancelled appointments with less than 24 hour notice for the full contracted rate of their appointment. Insurance plans do not cover missed or cancelled appointments. You will be required to sign the financial agreement prior to meeting with Dr.Putcha.
Insurance
If we do not accept your method of insurance, you will be required to Self-Pay for your visit and we will gladly print a claim form for you to submit to your insurance company.
We currently accept the following insurance companies and will submit the claims for you:

Out-of-Ntwk Blue Cross Blue Shield - you MUST have OUT OF NETWORK benefits.

Pls note that Medicare pays apx. 55% and secondary pays apx. 45%. If you do not have secondary you will be required to pay the 45% of the fees at time of visit. Please make checks payable to Putcha Associates. Allow 5 business days for processing after remittance and consent is received.
